This is a sample Affidavit of Income for your SSS Educational Loan application.

You need this if you are a Self-Employed or a Voluntary member, and you cannot present a document that shows the amount of your monthly income.

You need to have this affidavit notarized. Look for Notary Public desks near schools, city halls or SSS branches, as these are more affordable.  There are even local officials that offer free Notary Public service as part of their services to their communities.

Remember:  Your monthly income should not be higher than 25,000 pesos a month, and that your last SSS salary credit should not be higher than 15,000 pesos.

Note:  In addition to this affidavit, SSS might ask for a sample sales receipt, slip for release of product, or agent ID, so bring your agent ID or a sample receipt.

                         AFFIDAVIT OF INCOME

I, MARIA MAGANDA, of legal age, Filipino, married, and presently residing at 888 Masipag Road, Barangay Kasiyahan, Quezon City, after having been duly sworn in accordance with law, hereby depose and say:

1.  That I am earning money by marketing and selling various beauty and health products directly to customers as a direct marketing agent of Kerasha, Prima Collection and Beauty Pillows.

2.  That my monthly income ranges from 6,000 pesos to 10,000 pesos.

3.  That I am making this affidavit to comply with one of the documentary requirements of the SSS Educational Loan program.
